Payments
Payment is due prior to services being rendered. Credit cards will be kept on file to bill for co- payments, co-insurances, no shows and cancellations. Please contact your insurance company to verify/confirm benefits, especially out of network benefits for outpatient mental health services.

Out-of-network insurance
If your insurance is “out-of-network”, we will provide a detailed receipt, called a “superbill” which can be submitted to your insurance company for possible reimbursement.
Please consult with your insurance company for any questions prior to your visit.
We accept all major credit cards, Cash app, Zelle, Venmo, PayPal, and HSA/FSA cards.
Private pay:

Initial Visit: $300 (60 minutes)

Follow-up visits: $150 (30 minutes)

INITIAL self-pay visits are charged full price ($300) of visit at the time of scheduling.

Other Fees
Patient initiated telephone calls:

5 to 10 minutes – $25

11 to 20 minutes – $50

21 to 30 minutes – $75
Scheduling or calls initiated by your provider are excluded from this charge. This is not usually billable to insurance.

Medical records preparation (paper copy) – $25 flat fee

Emotional Support Animal letter/additional forms – $100.00 flat fee

Disability forms/paperwork – $125.00 for each submission of paperwork
A valid credit/debit card is required to be on file when setting up an appointment for all patients excluding Medicare and Medicaid.
